Wellbeing Service

The Wellbeing service is carried out by psychiatric nurses who have a registered mental health nurse qualification. This is run within our practice and appointments can be booked directly via our reception.

They offer thirty-minute appointments (face to face or telephone) to assist in managing mild to moderate Mental health concerns. The Care that is delivered is patient centred, safe and effective, consistent, high quality and compassionate. Referral to voluntary agencies, Primary Care Mental Health Team and Community Mental Health Teams can be considered.

 

Interventions can include the following.

· Single question conversations

· Advice on self-management.

· Screening feedback

· General advice

· Information on self-help resources including guided self-help including e-therapy.

· Accessing cultural resources

· Access to social, financial, housing, and educational and employment services by providing information and sign posting.

· Supporting people to make informed choices about their lifestyle and impacts on their mental health

· Motivating people to take steps that are effective in the primary prevention or delay of cognitive decline or dementia.

· Providing personalised alcohol and drug use reports with advice.

· Memory Screening

· Lifestyle advice.

· Motivational Coaching.

· Mental Health Risk Assessment.

· Psychotropic medication education

· Promotion of community asset building.
