Community Link Worker

Kat Hill is our Community Link Worker, based in the practice every Wednesday - Friday. She offers 30-minute appointments to discuss social issues impacting your life, providing practical support and advice, signposting to local and national services, and community-based groups and activities.  

 

This includes issues such as housing matters, your mental health and wellbeing, financial concerns, welfare and benefits, employment, bereavement, alcohol and drug use, physical activity and exercise, food and fuel insecurity, health worries, and social isolation.
